School Sport Physical Examinations

School sport physicals are given in August of each school year. Appointments can be made through the nurse’s office. The sport physical examination is in effect for one full school year. However, if the physical was given more than thirty days prior to the season, a medical update form also needs to be completed by the parent/guardian.
