The Nurse’s Office is concerned with the overall health and well-being of the student and how his/her health impacts the educational process.

It is important for the parent or guardian to keep  the nurse informed of any health concerns. All information will be treated with sensitivity and confidentiality. For matters of an urgent nature that need immediate nursing attention please contact the nurse via telephone rather than by email.

********Important New Legislation From New York State Regarding Vaccinations:
On June 13, 2019, Governor Andrew M. Cuomo signed legislation removing non-medical exemptions from school vaccination requirements for children.
As of June 13, 2019, there is no longer a religious exemption to the requirement that children be vaccinated against measles and other diseases to attend either:

  • public, private or parochial school (for students in pre-kindergarten through 12th grade), or
  • child day care settings.

  • You must have written permission from a parent to be examined by the school doctor.
  • Please note there is a place for parental permission on the bottom of page 2 of the Health Examination Form.  
  • Please see the Nurse to schedule an appointment
  • Sports Update Forms cannot be dated nor accepted any earlier than 30 days prior to the start of tryouts.
