Parent Consent

     

A consent form, signed by the parent or guardian, is required before the student may participate.  This consent form contains such vital information as the names of the parents, code of conduct and any medical concerns.  It also includes the parental home and work phone numbers and the phone number of an adult the school district can contact in the event of an emergency.  This consent form must be on file in the nurse’s office prior to the start of each season.
